This one is for initial application for licensure in Florida. If you are not NCCPA certified you will be issued a temporary license until you are certified. If you are already NCCPA certified you will be issued a regular license.
This one is for initial application for prescribing privileges in Florida.
This form must be submitted within 30 days of adding or deleting supervising physicians who delegate prescribing privilges to you, or if adding or deleting practice site locations or satellite locations.
