FAPA Membership Application Forms

If you are not a member and would like to join FAPA to take advantage and see more of our online resources such as the Members Only section, the PAper (newsletter), and CME links, please select an application method below. New members can expect to receive their membership packet in 3-4 weeks after your application is received in the office and payment confirmed.
 
Membership dues year runs from January 1st through January 1st.
 
Current members can renew online, or choose to download and print the form to fax or mail to the FAPA office.
